For your first appointment, please arrive 10 minutes early to fill out necessary paperwork. Paperwork will include a health history form that asks: social security # of you and your dependents, past medical history (you and your immediate family), current medications, dates of immunizations and current health concerns. You will also need to fill out a short insurance form and present your insurance card so a copy can be made for your file.

Whether you are transferring from another clinic or relocating to the area, it is always helpful for our physicians to have access to your past complete medical history. Therefore, it is in your best interest to contact your past clinic and have your charts released to us. It is your responsibility to sign the release waiver from your past clinic and forward your medical records to us.
